Are you a healthy patient between the ages of 40 - 65 who is being referred for a screening or surveillance colonoscopy? You may be eligible for direct scheduling of the procedure, without a pre-procedure visit. We would like to offer this convenience in scheduling for our healthy patients.

Please complete the Online Open Access Colonoscopy form. By clicking the button below,you will be redirected to a HIPPA-secure page where you can fill out the form and it will be sent to our office securely.

Additionally, after you have completed the form please click on the Appointments tab above and send us a message to notify our office that you have submitted the packet, and make sure to pull down "Open Access Colonoscopy" from the "Appointment Type" menu.

Our office staff strive to efficiently review and respond to all Open Access Colonoscopy requests within 5 business days. You will be contacted to review your information and schedule the procedure, or to make a consultation appointment if based on the information provided an office visit prior to the procedure may be beneficial.
